Summary
"It is just an age-related problem." This is one of the most common reasons women dismiss persistent hip pain. They feel that this pain is an outcome of aging, menopause, and a hectic life, hence neglect the issue without consulting an orthopedic doctor. While aging does not indicate the need for hip replacement surgery by itself, persistent pain that interferes with climbing stairs, walking, sleeping through the night, and performing other daily tasks may be a result of serious damage to the hip joint.
According to Dr. Sumit Anand, one of the top ortho doctors in Gurgaon at Miracles Healthcare, "Women often try to cope with hip pain rather than address it on time, considering it as a normal aging problem. But this should not be tolerated; otherwise, the cause of this pain can be determined earlier."
Why Do Women Have More Hip Problems?
Compared to men, women are more likely to develop hip conditions that can lead to chronic pain, reduced mobility, and eventually, hip replacement surgery. Research published by the National Institutes of Health (NIH) shows that women have a significantly higher lifetime risk of developing symptomatic hip osteoarthritis, 28.6% compared to 18.5% in men. In addition, hip fractures are estimated to occur about 3 times more commonly in females, showing the greater effect of hip disorders on their health. While longer life expectancy contributes to this increased risk, biological factors such as hormonal changes after menopause, lower bone density, differences in hip anatomy, and autoimmune conditions play a much bigger role in the health of a woman's hip joint.
"Women often attribute persistent hip pain to aging or menopause, but early evaluation is important because timely treatment can slow joint damage and improve long-term mobility," says Dr. Sumit Anand
Some of the main factors why women tend to have hip problems are as follows:
1. Osteoarthritis After Menopause
During menopause, estrogen deficiency can negatively impact the cartilage and bones, making them vulnerable to osteoarthritis. With the wearing down of the protective cartilage, the bones start rubbing against one another, resulting in pain and stiffness. Osteoarthritis is the most prevalent cause that makes women need hip replacements.
2. Hip Dysplasia
Another factor that makes women more susceptible to hip issues is having hip dysplasia. It means that the woman is born with an underdeveloped hip joint.
This misalignment results in unequal strain, which in turn results in arthritis earlier in life.
3. Osteoporosis and Low Bone Density
Females are more likely to suffer from osteoporosis after they reach the stage of menopause. Low bone density is responsible for weakening the bones and increased hip fractures, along with rapid deterioration of joints.
4. Auto-immune Disorders
Some immune disorders like rheumatoid arthritis are seen in women more than men. Continuous inflammation leads to damage of the hip joint.
5. Changes During and After Pregnancy
Hormonal changes, increase in weight, and postural changes during pregnancy increase the strain on hips temporarily. While pregnancy does not cause hip replacement, these factors could worsen any hip problem.
6. Increased Life Span
The life span of women is usually longer compared to that of men; hence, more time is available for age-related joint problems to develop. This explains why more women undergo hip replacements, especially among older patients.

Common Symptoms of Hip Replacement Surgery in Women
Hip pain rarely becomes severe overnight. In most women, the symptoms develop slowly and begin to interfere with day-to-day activities long before they consider seeing an orthopedic specialist. The problem, which may begin with just some discomfort, may slowly become more complicated over time, making everyday activities more complicated and having a negative impact on a person’s independence and confidence.
In case you suffer from any of the following hip replacement causes, it would be a good idea to visit an ortho doctor near you for a consultation.
1. Persistent Hip or Groin Pain That Doesn't Go Away
One of the first symptoms that can indicate that a person suffers from this condition is the feeling of persistent pain in the hip or groin that does not go away even during rest, after using painkillers, or after visiting a physiotherapist.
You may have
-
Pain when carrying your child or grandchild.
-
Pain when being in the kitchen cooking.
-
Hip pain after shopping.
-
Taking breaks frequently while walking.
2. Difficult Movements Caused by Hip Joint Stiffness
As the health status of the hip joint worsens due to the development of arthritis, stiffness may begin to cause trouble with movement, particularly when the person wakes up from sleep or stays in one position for some time. It may become increasingly difficult to perform once easy actions.
You may experience
-
Problems getting up from a chair or couch
-
Problems sitting with legs crossed while praying or meditating
-
Problems bending over to put on leggings, socks, or shoes
-
Stiffness when getting out of bed in the morning
"Persistent stiffness that restricts normal movements is frequently a symptom that the hip joint no longer functions normally and should be examined," says Dr. Sumit
3. Difficulty Walking or Climbing Stairs
A woman is more likely to detect hip joint pain during her normal daily activities rather than through exercises. Walking in the market, going up or down the stairs in your home, or following up with your family might pose increasing challenges with your hip joint condition.
Signs include
-
Hanging on to the stair railings each time you ascend or descend the steps.
-
Walking slowly due to pain in the hip.
-
Taking breaks when walking in the market or mall.
-
Being unstable and having a limp by the end of the day.
4. Household Chores Become More Difficult
Hip pain can cause household activities to be difficult. Standing, bending, carrying, or moving about the house can start to become difficult.
Some symptoms that you might recognize include
-
Pain when preparing food and standing in the kitchen.
-
Pain when sweeping, cleaning, and doing laundry.
-
Pain during the process of bending to clean the lower shelves or picking up things.
-
Sitting frequently while doing house chores.
5. Caring for Your Family Becomes More Difficult
For many women, the first signs of advanced hip problems appear while caring for loved ones. Regular caregiving tasks may become painful because hip mobility decreases.
You might observe
-
Difficulties in carrying your baby..
-
Discomfort in assisting your kids to bathe and dress up.
-
Avoidance from activities that require bending and lifting.
6. Getting Dressed is No Longer Easy
Lack of hip flexibility will make the simple process of getting dressed quite difficult.
You will observe:
-
Unable to wear a saree, leggings, jeans, or salwar.
-
Experiencing pain while putting on your shoes, sandals, or socks.
-
Unable to cut your toenails.
-
Unable to sit crossed-legged while praying or meditating.
7. Hip Pain Disturbs Your Quality of Sleep
Night time is when the symptoms of hip arthritis become most evident and impact your sleeping patterns.
You May
-
Get pain when sleeping sideways.
-
Have to wake up every time there is movement in bed.
-
Constantly shift positions to get comfortable.
-
Feel tired the following day due to lack of restful sleep.
8. Loss of Independence
The most obvious sign of hip pain being something more serious is when your independence starts to be affected.
You will observe:
-
Giving up yoga or morning walks due to pain.
-
Not attending family gatherings or holiday shopping.
-
Difficulties with travelling.
-
Depending on family members for daily activities that you once managed on your own.
-
Feeling less confident leaving the house because walking has become painful.
"Women often put their own health last, adapting to hip pain instead of seeking treatment. If pain is limiting your daily routine, mobility, or independence, it's important to get your hip evaluated before the joint damage progresses," says Dr. Anand

Average Age of Female Hip Replacement
There is no fixed age for hip replacement surgery, but most women undergo the procedure between the ages of 60 and 75 years. However, women with severe osteoarthritis, hip dysplasia, rheumatoid arthritis, avascular necrosis, or hip fractures may require surgery earlier, while others may not need it until later in life.
Rather than age alone, orthopedic specialists consider the severity of hip joint damage, pain levels, reduced mobility, and how much the condition affects daily activities before recommending hip replacement. If persistent hip pain is limiting your independence or quality of life despite medications and physiotherapy, it may be time to consult an orthopedic specialist.

Frequently Asked Questions
Pain is usually felt deep in the groin, but it can also spread to the outer hip, buttock, thigh, or even the knee. The pain often worsens with walking, climbing stairs, or prolonged standing.
The one-leg test assesses balance and strength of the hip muscles. While difficulty in balancing on one leg may indicate a hip injury, this does not prove the need for a hip replacement. This test needs to be done along with an orthopedic assessment and imaging.
Hip replacements are most commonly done at the age of 50 to 80 years. However, it depends upon the level of joint damage, degree of pain and restriction of movement rather than being age related.
X-ray can detect loss of cartilage, narrowed joint spaces and bone spurs that indicate the presence of hip arthritis. The need for a hip replacement surgery is dependent on the results of the X-ray and your symptoms.
Pain most commonly starts in the groin, but it may also be felt in the front of the thigh, buttock, outer hip, or knee. Many people mistakenly believe they have a knee problem when the actual source of pain is the hip joint.
